These are the collection achievements that you need to complete to get the 3 exotic weapons.

Healing Heir (Sword)

How to unlock the collection achievements?

To unlock the collection achievements, simply zone into Lion's Arch. You will receive an in-game mail that tells you what to do next.

Throw these items into the Mystic Forge to get Ectoplasm-Infused Vision Crystal:

2 Lesser Vision Crystals
3 Vials of Blood
6 Sapphire Orbs
15 Globs of Ectoplasm

Consume the Ectoplasm-Infused Vision Crystal. This will allow you to see and interact with dormant souls. It is also the 1st item in all 3 collections. The 3 Rare Collections achievements are now unlocked.

If you are going to get all 3 weapons (Torch, Greatsword & Sword), craft the Oiled Orichalcum items beforehand. Craft:

Oiled Orichalcum Torch Head (Huntsman),
Oiled Orichalcum Greatsword Blade (Weaponsmith),
Oiled Orichalcum Sword Blade (Weaponsmith)

Legend Builder

Description: Help Bakkin earn renown and fire the Favor of the Colossus
Rewards: 7 AP + chest + Favor of the Colossus (exotic torch)

Warming Up
First, in Dredgehaunt Cliffs: Find warmth at hearth close to Hoelbrak.
Port to Graupel wp [&BGICAAA=] & head east into the house. Talk to Bakkin.
Transformation
Port to Travelen's wp [&BGQCAAA=] & head west, across the bridge to Leopard's Snarl Shrine PoI.
Start to Stop
Port to Steelbrachen wp [&BFsCAAA=] & head east into the caves. Go down the stairs.
Larger than Legend
Port to Dociu wp [&BFgCAAA=] & head southeast to the Hero Point.
A Breath of Life
From Tribulation wp [&BFYCAAA=], head southwest to the tower & climb to the top.
Imbued Oiled Orichalcum Torch Head
Return to Bakkin at the first location at Brandalf's Steading PoI (Graupel wp [&BGICAAA=]). Make sure you have the Oiled Orichalcum Torch Head before talking to Bakkin.

A Shade's Blindspot
From Kestrel wp [&BD0EAAA=], head south to the Hero Point.
Memory Lane
From Helliot Mine wp [&BEsBAAA=], head north to a cave where the Guild Challenge is.
Blame Game
From Skoll's Bivouac wp [&BEoBAAA=], head to the big tree in Thunderbreak Hills, north of the heart.
A Balanced View
From Fangfury Watch wp [&BEwBAAA=], head southeast to the waters in Highden Caves.
Ghost Growth
From Ogre Road wp [&BE8BAAA=], head straight south, right to the end.
Imbued Oiled Orichalcum Greatsword Blade
Return to Xeniph at the first location at the Hero Point, south of Kestrel wp. Make sure you have the Oiled Orichalcum Greatsword Blade before talking to Xeniph.

Healing Heir

Description: Mend Nayeli's splintered soul.
Rewards: 7 AP + chest + Scion's Claw (exotic sword)

Location: Dry Top

Scattered Self
From Dry Top Entry wp [&BIAHAAA=], head west to the vista on top of the cliff.
Updraft of a Dream
From the previous spot, head west around the hills, near the vista.
Duty's Shards
From Repair Station wp [&BJcHAAA=], head southeast towards Cragrock Palace PoI. Xeniph is in an underground cave, south of the PoI, by the Hero Point.
Low Spirit
From Vine Bridge wp, head north to the Hero Point in Upland Oasis.
A Soul Resewn
From Repair Station wp [&BJcHAAA=], head south. Xeniph is on a ledge at the side of the cliff.
Imbued Oiled Orichalcum Sword Blade
Return to Nayeli at the first location at the vista, west of Dry Top Entry wp [&BIAHAAA=]. Make sure you have the Oiled Orichalcum Sword Blade before talking to Nayeli.
